I just did a 6 bolt swap and the car is doing crazy things.
1st start idled great at 850
2nd start (2 days later) - RICH! There was a lot of exhaust and it smelled super rich
3rd start (25mins after 2nd start) - LEAN and hardly any exhaust.
4th start (3 hrs later) - Super rich again.
Wideband (downstream) was around 14.5, then dropped to 19(max) on the gauge during run 1
My vacuum is at 15-18Hg
Mods List is up to date. 560cc injectors, E316G, FMIC, Tuning with ECUflash

@m_0ney_pit I used Ceddy's Mods - Scaling was 565 (cc injectors) and latency is set to -.048, but looking at it I put in +.048. here is my scaling, latency, and MAF charts:

I am going to upload this new tune tomorrow and see how that goes. If there are any other changes you would suggest, let me know.

Have a look at the thread were Tom from ecm tuning tested the evo 560cc injectors. he found that if they are used with the DSM injector driver the latency must be increased 10usec but with the evo injector driver the latency is actually lower. what I don't know is if the injector driver is the same between 95 ecu and 98-99
 
@m_0ney_pit Ok I just did the test run, started lean, then went back to around 14.7-15.5, then back to 19, but it was idleing great at 850rpms consistantly, but gauge said 19

Edit: The time it took to go back to the normal range was about 1 minute, then another minute or so to shoot back to 19.

Edit 2: It started off with normal exhaust, then when it went to the normal afr range it was pouring out exhaust. Then at 850rpm idle, it was normal exhaust again.

I have a suspicion that my gauge is garbage.

Absolutely you need to get EvoScan man. Not sure how you're tuning without it. It's only $25 last time I checked. There's this great feature called maptracer, which makes tuning much easier.
 
So I just changed my fan temps on my ecu and I gave it another try. In closed loop (start up fuel map if I remember correctly)
it sat at 14.6-15.1, then when it dropped into open loop, it went lean. So I am going to add a bit more fuel because closed loop is supposed to be rich, and open loop is supposed to be at 14.7.
 
Closed loop refers to when the ECU deciphers your AFR based on the front 02 sensor feedback. Basically at idle (warmed up) and during cruise. Open loop refers to when the ECU deciphers your AFR based on the open loop maps, so when you are in boost basically. There is a sub menu in ECU flash where you can change when this transition (from closed to open loop) will occur. Sometimes it is a good idea to put your car in open loop at all times in order to trouble shoot and you can do that from this menu. This is why you tune for idle, cruise, and wot separate based on the above info.

I believe what moneypit was referring to is another separate table, which is for cold startup enrichment. This table decides how much fuel to inject when your engine is warming up and is separate from closed and open loop.

Awesome! I had to make a lot of changes, but I got it to hit ~16 at startup, 12.4 during the warm up, and 14.5-15 in open loop.
Vacuum is sitting nice @ 19HG. I'm going to burp the cooling system, check the trans fluid levels, and take it for a quick drive!
I have been reading the post about how to tune WOT, but I may need a bit of guidance. I will let you know. Thank you for all of your help. PS also switching to an AEM wideband due to some funky electrical issues with the APSX WB.
