I took off my valve cover to replace the gasket and sparkplugs. When it was off i noticed that both rockers in one spot fell off and were in the engine just floating. i looked and nothing seemed damaged. The rockers are in good shape and the lifters seems good. I dont know what made them fly off.
 
A few things could be the reason why,

A lifter thats not pumping up enough,
Cams thats have been reground,
Lifters that are not shimmed enough,
Just general wear and clearances get a bit bigger,
High revs and do this also seems alot more on the intake side since its pushing the rocker forward (this rocker has less chance to stay on) due to its angle and direction of the cams rotation
