Whats causing my low vacuum?

I've been searching like crazy and I've replaced every vacuum line that I can think of. I'm still only getting 14-16lb/hg at idle. If I hold the brake pedal down while coming to a stop the car wants to stall out. Is there a specific spot that would cause the car to lose vacuum especially when the brake pedal is pumped? It also causes the car to lean out.

you sure you checked every vacuum line,especially around the intake manifold/throttle body area

Yep, I've even capped them all off. I do have a GM MAF with a MAFT translator on it. Could this cause my idle to be low or my vacuum to be low. The car also seems to spit at idle, like it has a slight miss in it.
