Problem with clutch fork.....

This is the 2nd time this has happened, and is really driving me nuts. I'd would be driving along normally as usual when my clucth slams and sticks against the floor. I know what it is b/c it happened last time. The slave was leaking and the clutch fork snapped in half. What can I do to fix this issue so it doesnt happen again?? I have a 2600 in my car and adjusted the adjustment rod in the pedal. I want to make sure I order everythign and replace everything as needed this time. I ordered the push rod from Taboo, slave cylinder and clutch fork for conicelli (since I was told teh fork was revised). Is there anythign else that I would need???? I know the clutch is still good too. Thanks guys.
 
Any ideas??? I just dont want to pull this trans again if it happens for the 3rd time. :cry:
