Is it the clutch or Transmission? (pics)

So I finally was able to remove my tranny and take a look at my clutch.
Here is the previous thread: http://www.dsmtuners.com/forums/drivetrain-tech/439976-twin-disc-engagement-issues.html Pretty much my clutch wasn't dis engaging.

My original guess was that I was over extending the pressure plate fingers and causing it to not fully dis engage. Now looking at it I can't tell if that's the problem I have.

Here are various pics showing:
Machined clutchfork
TOB and Clip (showing range of movement)
Twin disk fingers and flywheel

Does the clutch look consistent to what over extended fingers would do? I replaced my clutch pedal assemble and re bled everything already.

There is also a pic of my axle seal, Is it salvagable?

I'm about 420a's, but your clutch looks pretty good. Someone correct me if I'm wrong... When you say the clutch disengages, do you mean it disengages after you have the pedal to the floor? Like it doesn't engage at all?

never mind, i read it wrong... so after you engage the clutch, it doesn't disengage?

Correct, When I would press the clutch I couldn't go into any gear as if the clutch never dis engaged at all.

You guys seriously missed the front clutch disk hub missing all of its splines and the input shaft spline damage??????

Take apart your clutch assembly. Your disk pack is junk.

Holy Toledo! I just noticed this in the pics! Good old Tim with the save. Can all this be attributed to over extending the pressure plate from not adjusting clutch properly?

The big thing for me is learning why it did this so I won't repeat my error. Only 600 miles on twin disc and shep stage 3. Clutch just stopped engaging one morning but I always heard excessive noise when pressing clutch pedal (over extending). Replaced clutch pedal assmbly, re bled and still nothing and now I'm getting a sbc and fixing tranny. Don't want the same thing to happen again
 
Over-adjustment, or if there was a shimmed pivot ball in the mix, that would cause havoc on a QM twin.
