car hops forward when clutch is engaged...wtf!?!?

I was at a red light and i had my clutch engaged to the floor and i gave it a lil gas to about 5 grand and the car hopped forward ihad a act 2100 installed and i have put about 1k miles on it! also my clutch has been slipping coming out of second into third at wot. im not sure that this is a normal thing? can anybody help me out with this ?!?!? thanx!!!
 
how old is everything? meaning, has anything been replaced? it sounds like the clutch isn't DISengaging all the way (what you accomplish by putting the clutch pedal to the floor). you might have to adjust your master cylinder push rod a little bit
 
my tranny went out on me a lil while ago i had a rebuilt one put in along with a act 2100 and a quaife lsd everything is new i tried adjusting the clutch but i guess i didnt turn the rod the right way if anybody could help me on this i get confused on which way does what on the rod please help thanx!!!
