weird lean issue

When driving at times PLX wideband will read from 17-19:1 super lean. The car loses power and runs rough. The only way to stop this is to shut the car off and restart The CEL comes on and the faults are:

Front o2 sensor heater circuit malfunction-which isn't hooked up but dsmlink still reads front o2 volt
EGR flow malfunction- i have a blockoff plate
Random misfires detected


I had this problem a couple days ago then it went away the next day. Now its back, I need some input on what should I do to fix this.
 
What's the info on mods on your car.. Need info like AFPR or not, Pump size and brand, size injectors, tuning method or unit. If there was anything changed or some wires or something messed with right before it began we need to know to help. is the pump wired? when was the fuel filter last changed?.. fill us in.
