Is there any reason why my OEM TOB completely fail (fell apart basically). This is the second time this happens about a year apart. I don’t have the rubber boot that goes on fork, could it have dried out and caused premature total failure? I must note it’s been rattling since I first installed it. yes mitsu OEM
 
If you over adjust the master you close off the port that let's fluid move back and forth when static. At that point the system does not auto adjust as the clutch wears and gets thinner. If you can push the slave in by hand it's fine.

The master is actually maxed out BUT I’m I didn’t shim pivot ball, I did verify that I was able to push slave in by hand towards driver side after I bled and adjusted
 
I can't see that causing it. Something uneven. Possibly scored bellhousing snout? Broken pressure plate finger, although that would likely cause other symptoms
