Ok, I pulled the tranny from my car tonight. The issue was not getting complete clutch disengagement. I installed a new clutch master and slave, bled the lines and slave with the help of a speed bleeder.
This is what the clutch fork looks like, obviously worn?:

I already know the clutch disk needs replaced, it's nearly worn down to the rivets. I weighed the flywheel, it's about 18 pounds. Seems it's full of cracks, I wonder if RRE will take it in for core exchange for their 14 pound flywheel.

get a whole new clutch and flywheel kit

Yes, I went ahead and purchased a Gripforce stage3 clutch kit and 11 pound flywheel. It's installed now with new clutch fork/pivot ball, clutch master/slave and bled the heck out of the lines. The tranny is still stuck in gear, clutch disengagement is perfect. Seems like the tranny is stuck in 5th gear. I'll have to pull the tranny again and remove the gear stack for inspection.
