Grinding in every gear, what could it be?

I did a search and got like 10 different answers. Anyway I just picked up a 90 Galant GSX AWD with 169k on it. The clutch pedal feels very soft like it has no pressure. When I drive the car it grinds in every gear unless I ease it into gear VERY easily. I looked at the master/slave cylinders and they are not leaking. I changed the tranny fluid to Pennzoil Syncromesh, bled the clutch system and still no difference. What else can I try? Could it be the clutch itself going out? If so how do I test it. If anyone has any thoughts on what it could be please let me know. Thanks
 
Oops forgot to add, I've also tried adjusting the cluch pedal and this didn't help either.
