What would cause this?
This head was rebuilt only 2500 miles ago.
I was going to put some new valves in the head this week end.
But I decided this cam cap is way too messed up to try and reuse this head.
Now I guess I'm going to have to order another head...
But why would this have failed so prematurely?
Every other cap looks mint.
 
Or the wrong cap was installed in that position.

But it needs to be fixed before it goes back into service.

How thick is the head? Did it have oil port mod 1 done?

That is #1 Intake, and the furthest away from the oil feed to the head.
 
No oil port mod done to this head.
The head just had a valve job and resurface.
But one valve is missing a piece.
So I was going to put some new valves in the head.
But machine work up here is so expensive it is probably going to be cheaper to buy another stock rebuild head.
