1G What kind washer timing belt idler pulley?

Idler pulley somehow spun off. I must not have torqued it correctly. I'm guessing it was just a flat washer? Checked the bolt thread and it wasn't in there.

Idle pulley came off and youre worried about the washer? What about all the bent valves?

It's a "washer assembled" bolt, meaning it uses a lock washer.

Lol I guess the angry face didn't convey my fury for this ruining my two-year-old top end. Head is headed to the machine shop. I do want to get the correct washer because I can't afford to go through this again!
I rebuilt the engine summer of 2014. I can't believe I didn't torque it correctly the first time! I was very careful about everything I did. It was my first rebuild and I think I did pretty good to get a year and a half of daily driving and road trips without any major problems.
