oxygen sensor high volts

What would cause my oxygen sensor to read over 2v in closed loop. Is it bad? Could a bad ground be the problem?

Make sure the O2 sensor signal ground wire is connected to ECU pin 92 - NOT to ground (it must be connected to ground ONLY inside the ECU on pin 92 - NOT anywhere else).

Ok, I'll check that when next time I work on it. When I was putting in my ecmlink chip and WB it looked like someone did some splicing on some wires..mostly black wires on the same wire group as pin 75.
