1997 AWD, ACT 2600 clutch, ACT flywheel, mitsu factory T/O bearing.

It looks like my transmission is eating another throwout bearing. I had an issue about 2 years ago where this same thing happened. I'm now suspecting that there's another root issue that's causing it to eat them, what should I be looking for?
