Eating another throwout bearing?

1997 AWD, ACT 2600 clutch, ACT flywheel, mitsu factory T/O bearing.

It looks like my transmission is eating another throwout bearing. I had an issue about 2 years ago where this same thing happened. I'm now suspecting that there's another root issue that's causing it to eat them, what should I be looking for?

To look for idk , i would pull it apart and check the drivetrain system.

Transmission shop thinks it might actually be the center diff. I'll get an answer tomorrow after they get it up on a lift.

Update, it's coming from INSIDE THE TRANSMISSION.

Time to get some rebuild quotes, I guess. Don't think I'm up for R&R'ing a transmission in my driveway,...

No, it wasn't the clutch fork.

Mine has gone through its second one in 3 months, i was told it was from not resurfacing my flywheel. Everything else was new though.
