I think you also need a compressor housing. Exhaust housing+center cartridge+compressor housing; of course you need feed lines for oil and/or coolant and their respective return lines. I'm probably missing something but you get the idea. I'm pretty sure that it won't be more cost effective to build a turbo the way you are talking about but price it out and see.
 
Yes, you should get it spin balanced once you are finished. I think it's going to end up costing you around the same price in the end as buyin it new, and you'll have all that time in searching and assembly that no body is paying you for.
