W5M33 Evo II GSR rotational noise in 5th gear

I replaced the reverse synchro because the last one was terribly scarred. 5th gear has a loud rotational sound. Any ideas what it could be? Below is the video of it on jackstands. I fixed the issue of the reverse grind but looks like there's another issue.

Did you check the end nuts and make sure they are tight?
Is this unit new to you? Rebuilt? Recent work? Has it always done this or just started?
What you need to know is did something break or did somebody put something together wrong.

I tightened the end nuts and knocked the indentations in. I've had the vehicle for 3 years and this rotational sound occurred after it sitting for a week or so. I had previously done the nut tightening thing but I didn't knock the nuts into the indentations because I didn't realize how to. The main shaft nut was loose when I opened the case this time. This sound is completely new. Prior to me opening the case this time the 5th gear lever would move on accel/decel and reverse would grind going in every time.
