Too much black/blue smoke

I have run seafoam several times with good performance results, but the smoke soon comes back. The smoke is mostly black. Is there any way to get rid of this? I've thought of replacing the muffler, but feel like that will only provide short term results. Does my car need tuned? 95 Eclipse GST w/ no mods.
