terminal turbo problem

I was driving around today and the car started to act funny. The boost was erratic first it would spike to 20 psi then drop to 15 then back to 18. Then the real fun began with a pop and the sound of something metal bouncing around. I babied the car home and started with pulling the plug, but the were good. Then on the intake. When I pulled the intake off In knew it wasn't going to be good when some thing metal fell out of the intake an hit the ground. The nut lt for the impeller shaft had come loose and bounced around into the impeller taking out most of the blades. I guess it'll be a good excuse to get an EVO III 16g . Any way is this a common problem for this to happen? I have never heard of this happening before. Is the supposed to lock-tight on the nut?

this is something that very rarely happens, but does happen.
