Dropping a tranny in a 90 AWD?

Hey guys I had a couple quick questions for everyone who's removed a 90 AWD 5 speed tranny.

I used to own a 92, and dropped the tranny out of that car, and installed an ACT 2600. The 90 I just recently bought has an ACT2100 in it, and I'm pretty sure that I busted the throw-out bearing doing some launches the other day.

Its making that "chattering/rattling" noise coming from the transmission - the pedal has also felt different than in the past.

Now, this ACT2100 clutch was just put in by MISTU (yep, for $550) they guy gave me the receipt. This was only about 5 months ago, and I imagine they put in a new T.O.B. then. What would cause this new T.O.B. to break so easily?

I also picked up axle seals, and a new OEM throw-out bearing from the dealership - is there anything else I'm going to need to replace dropping the tranny?

And I asked the previous owner if he had the flywheel machined properly and he ddn't know what I was talking about. Do you think the size of the step on the flywheel, might be causing something to not work properly?

When a T.O.B. fails, does it usually take any other parts with it?
