Broken Clutch???

Well, i live in Michigan and its about 5 degrees outside and of course with my new rebuilt trans and new clutch (10,000 miles) something breaks. I love my dsm but i hate the almost consistant work i have to put into it. Well, i was driving casually and all of a sudden my clutch stopped working. I had pressure on my pedal but it couldnt put it in gear. I slammed in the gears to get my car home. I dropped the trans the other night and pulled off the pressure plate and the entire middle of the clutch was destroyed, it was like someone put a bomb in there :thumbdown . Personally it wasnt a big deal, lifetime warranty clutch but im just seeing if you guys have any ideas why it broke, so i can prevent it from happening again. I did use the clutch allignment tool so that wasnt it. One thing i am thinking that happened was my trans shifted and made the clutch get out of line. Another problem i am having is too much vibration. I torqued all my bolts on my trans good and somehow after this clutch problem happened i lost both of my trans mount bolts and a starter bolt came out annnnd a bell housing bolt went missing. I am going to use lock tite when i reinstall the trans but im just wondering how i can fix this for good.

Check for missing dowel pins inbetween the trans and engine... Sounds like that is what did it.
