blown headgasket second time in 2000 miles

well a friend of mine blew his headgasket out. so we fixed it and now its blown again. everything was torqued to specs and done right. the problem is there is a hole in the block and the head that line up but not one going through the gasket and it was blown through on the old gasket. and the new gasket is the same with no hole. so should we punch a hole through it or should it be that way ? its by the #4 cylinder on the far side(not the front or the back) and there is 2 the exact same size but only the one towords the front of the car was open on the gasket.

and the gasket blew the exact same spot as the first on the #2 cylinder towords the back left corner.

the car is a 95 eclipse GST.
please reply ASAP the car is ripped apart in the garage and it needs to get put back together ASAP.
 
It sounds to me like there needs to be some type of bolt there, but I can't really tell w/out pictures.

Have you done a compression test on the car? How many lbs of boost is he running and does he have ARP's? What type of h/g are you guys using?
 
its a top line headgasket. hes only running 15lbs of boost. the fist time he was running 18 and turned it down. it had a pretty big burn mark in it was like 1/2" long
