what causes p/s pump clack?

my power steering pump sounds like a train running over the tracks, clack, clack, clack. it was only doing it when cold but its now happening almost all the time. what inside the pump would cause this? can it be fixed with a rebuild kit, or just as simple as a fluid change? fluid is not low.

It sounds like a fluid change won't help, and something mechanical has given. I would replace it with a used one if you can find one from the classifieds.
