1G O2 sensor problem reading 0.0v

My o2 has been reading 0.0v, even after a swap. Im assuming the a wire is broken/disconnected. Can anyone tell me if there's a common spot they go bad, and also which wire is which? Im thingking I might re-wire it to the ecu but I need the information to do that. I've done a few searches without much luck.
 
The harness side round connector has pin 1 in upper left, 2 upper right, 3 lower left, 4 lower right.
Harness side: red (pin 3 - goes to mpi relay) and black (pin 4 - goes to ground) are the heaters. Black (pin 2 - goes to ground) is signal ground and white (pin 1 - goes to ECU pin 4) is signal.
Sensor side: Every after market O2 has different color wires. The 2 same color wires, usually white (but sometimes black), are the heaters (go either way - they measure about 12 ohms so you can easily find them even if they are not the same color). The other 2 are the signal and signal ground wires. If black and grey, try the harness black to sensor black and harness white to sensor grey. If it doesn't work reverse these two. Is pin 1 on the sensor plug, (looking at with the nub pointing up and notch on bottom) always the signal wire?
 
Not sure what you mean by "always". Always in respect to what? As I said the signal wire is the white wire on the harness side connector. I may have the harness side and sensor side pin layout reversed. The manual shows the pin # layout as I previously stated (with nub up and notch down) but doesn't say whether they're looking at the harness side or sensor side connector. After looking a little closer it looks like they are looking at the male side which I think is the sensor side. So the pin layout I previously stated would be for the sensor side not the harness side. Either way the white harness side wire is the signal.

Perhaps a 1g owner can verify which connector they're looking at (I looked in the DSM Backup Manual).
