Need some help. Slave,bleed,etc

Got a new sleeve/slave cylinder. Ive seen/ heard it both ways. Put it in, everything goes well on the install not hard. Put some fluid in the resovoir and get ready to bleed. Should I have greased the slave cylinder before with any fluid before hand? So as we are bleeding the clutch the little boot thing pops off, take it all off, and put it back on. Now we bleed it again, and the pedal gains pressure for ONE pump, the next pump it goes away. Im pumping it once, then bleeding. The little boot/nipple thing that extends out of the cylinder doesnt move. It just extends all the way out and continues to stay there, doesnt move or anything. Little nipple sits in the hole of the tranny holder. ( no idea what its called ) LOL. So is this brande new cylinder a bad one?
 
Also is the rod supposed to be bent or straight? Is it supposed to move in and out?
