did you see if the fork is in good shape?
Looking at the pressure plate it seems that one side was making good contact and the other side wasn't.
When the trans was installed were the pressure plate bolts torqued in the correct sequence with a torque wrench and did you gradually draw the pressure plate to the flywheel while torquing it?
When you removed the trans were there any bolts looser then the rest (mainly on the side that has minimal wear)?

Add my car to this list! Mine doesn't slip in higher gears or RPMs (like my stock clutch did) but I have to rev to 5000 RPMs to make the car move in 1st gear.
Strange part is that the car runs IN first gear with my foot off the clutch pedal when parked, it wants to move forward slowly like an auto trans.

Did this happen all of a sudden like mine? I felt something give and a snapping noise then it started slipping.
I have a 2100 with their 6 puck sprung hub disc. I went with a Fidanza 2.1, it appears that they changed their disk design to hold the springs better.
