Clutch Slipping / Clunking noise

I completed a 6 bolt swap about 3k ago and used a Fidanza aluminum flywheel and ACT 2100 for the drivetrain. The clutch has always engaged right off the floor, but I was told this is normal. No amount of adjustment on the back side of the pedal has affected the engagement, but the car seemed to be fine until today so I left it alone. As I was coming home today, however, I noticed after stopping at a light that I was having trouble accelerating. The RPM's were climbing normally, but the clutch was just slipping. I immediately backed off and babied the car home. The clutch will slip very badly in first and second gear, but is all right once I'm cruising. Also, I am hearing a loud clunking noise from the front of the car. To describe it, it sounds as if a motor mount was bouncing back and forth with no inserts to keep it centered? Just a loud clunk at random intervals. I was told the clutch lever arm may be bent or the pivot ball worn and that there is a window to check the lever arm to see if it is centered. Do these symptoms sound like a common problem, or does anyone know how to check this lever?

One last thing I thought of was when we were installing the ACT 2100, two of the bolts holding the pressure plate to the flywheel actually stripped out the threads into the flywheel. To remedy this, we just used longer bolts to reach the back threads. Is there a possibility that the pressure plate may be loose?? Any help is greatly appreciated, thanks...
 
I also had a very loud clunking noise droped the trans and the throut bearing was in pieces and i had crankwalk and my presure plate was all f..ed up. so check to see where it is coming from, pull the dust boot of the clutch fork where the slave pushes on it and see if anything comes out because i had few metal pieces come out of there. just listen where it is coming from. what kind of throut bearing did you use? i was reading on here that atc bearing is not the best one out there. just check and good luck.

My first thought was lose PP bolts, then as I read further and say you had issues with them, that makes me think thats your issue for sure. Pull the inspection plug off at the clutch fork and see if there are missing bolts or some floating around. This would explain both the slipping and noise.

For the posts I have read (as I just installed a new clutch) you don't want the clutch to engage right of the floor & this isn't how its supposed to be. You want it about 1/3 (to about half) the way up. The adjustment rod on the back of the clutch pedal is pretty sensative. If you aren't getting any change, maybe you are to far out of adjustment to notice any difference?
