420A Another misfire post - what have I missed?

Hi All,
This is my first post here. I have been helping a friend with his 1999 Eclipse GS 420A. It does not fire on #4, seemingly at all. I know there are lots of posts on here about misfires, but we have been through everything and can't figure it out. I was hoping someone might have an idea of something we missed.

We rebuilt the engine about 8 months ago. Ran fine until about a month ago when it started to misfire.
Misfire on #4 only. Can pull #4 plug wire and nothing changes. Can unplug #4 injector and nothing changes. Pull plug wire on other cylinders and it is noticable - nearly stalls.
Here is a list of what we have been through.
Rebuild - stock rebuild - no performance add-ons.
Compression - 208-210 lbs all cylinders
Leak down test - 4%
Replaced coil pack (previously replaced at rebuild)
Has new plug wires
Replaced plugs
Replaced cam position sensor
Plenty of spark from plug boot when removed and placed close to valve cover
Swapped plugs and wires around
Replaced injector
Recently replaced IAC valve, but I don't think that should be related
Has power to injector - reads about 14.5 volts
Injector resistance is about 11 ohms. I haven't looked up spec but others are the same
Checked injector plug with test light - pulses when cranked
Injector plug appears fine - not broken, cracked, or brittle and it is clean.
Since we are getting spark it seems to be injector. And there does not appear to be fuel getting into cylinder. Plug is not wet.

As you can see, we have tried to eliminate everything we could think of.
This is the only one of these motors I have worked on and I don't remember what it looked like inside the intake.
Is it possible to have something clogged between the injector and the cylinder?
Is it possible to have a new injector that tests fine but doesn't actually work?

When you say replaced injector what exactly did you do. I would have swapped two of the ones in the car and see if the problem moved.

My friend had already purchased a new injector so we replaced the old with a new. Having same issue with new one installed.
We are getting ready to pull injectors again. Will swap around and see what happens.
We were also going to test them for flow before putting back in.
And look for anything obvious in fuel rail and manifold.
