clutch or flywheel chatter/vibration/grind noise

Basics:

stock fwd flywheel (not new, yes stepped properly)
4puck sprung CC clutch
master/slave bled and adjusted properly
clutch height/adjustment done properly

Problem:

When I slip the clutch, most of the time, it happens when starting from a stop.... If I slip too much, it almost seems like the clutch is skipping off the flywheel and making a noise. The best way I can describe it is like what brakes on a bicycle sound like when they're dialed in really nice and they "grab" the wheel so well that they vibrate it and it makes a kinda chatter/hum noise. Well, my clutch/flywheel is making a similar noise. What I want to know is, a friend said that my fw surface is likely uneven from the 4puck being too rough on it... which would be fine.

I want some more opinions on what this could be. If any further information is needed I will be glad to post any answers that can help diagnose this situation. The car is drivable and has been this way for a couple weeks now... it seems like it is SLOWLY getting a little worse.
 
Sounds to me like a glazed clutch from excess heat. Mine does it. I burnt the clutch a couple of times. I noticed if im super easy on it for a couple of days it goes away. Probably because it wore the glaze down.As soon as i lay into it. It will start doing it again when pulling out and downshifting.Like said before the TOB and clutch fork should be inspected as well;)
 
Check the clutch release Fork and Throw out Bearing. This just happened to my friends SRT-4 and when we examined the bearing assembly, that was the problem.

I did notice my tob was clicking the other day... but I haven't heard it again since. I'm fairly certain that it's the tob clicking because my friend knew his was bad and was showing me what it sounded like when he pressed and depressed his clutch and how you could hear the clicking with the clutch out, but not in...

Anyway, what should I be examining the clutch fork for? I know they can break, but what am I looking for? Will it be bent/cracked? Also, what does the TOB do when it goes bad besides click? How does it actually make the clutch/flywheel react?

I plan on getting a Fidanza flywheel with replaceable clutch surface soon so hopefully I won't be dealing with a stock fubar'd flywheel much longer.... I don't think I will be pulling the transmission to check before I get the new flywheel in my hands anyhow. Also, I imagine the Fidanza will be a little lighter, will the difference in having less rotating assembly really be noticeable in how the engine revs up/down?

Look for wear on the fork where the TOB sits, or wear where the fork attaches. This will give the fork extra room to move, causing vibrations. When a TOB is bad, IMHO, it makes shifting and clutching VERY DIFFICULT. This difficulty could be from a number of other problems tho, as well.
