Clacking sound in 2nd gear

Ok guys this noise does not sound good. I am almost positive its coming from my tranny. The noise is like a slapping noise (clack clack clack) and gets faster as it goes up. The thing is that it only happens in 2nd gear; no other gear. Also even before it goes into 2nd gear you can hear the sound faintly. I had the car stopped and idling and "presented" 2nd gear (put the shifter towards it but didn't actually put it in) and you could already hear the slapping sound faintly. What can this be? I am completely lost.
 
I want to add that he car goes into 2nd gear fine and also drives fine in it. It just makes a clacking sound that gets louder and faster as the rpms go up.
 
Thanks for the reply but I don't think it is going out since it doesn't grind at all and still hold in 2nd gear fine. It's just making that clacking sound.
 
I have gotten a couple of suggestions. One was that maybe the hub/sleeve went. The other is that I chipped a tooth. Anyone think one or the other or both?
