Throw out bearing issue w/ QM twin

I put this topic in here since it get's the most traffic.

Parts: Brand new QM twin, new OEM TOB, MAP flywheel and grinded fork.

Issue is that it kills TOB's right away basically. Not sure what I'm doing incorrectly on the install, but I have now done my 2nd TOB in driving normally away from the Base Auto Hobby Shop. The clutch only has just over 50 miles on it now. What could be causing this? Thanks for the help!
 
The QM and PTT setups are nearly the same. I run the PTT setup. I suggest ordering the PTT designed TOB and pressing it in according to their instructions (you use the collar from an OEM TOB). Give that a shot and I feel it'll work for you. The PTT unit is a lot sturdier looking.
