Strange Clutch Adjustment Problem

I rebuilt clutch master cylinder last weekend and all went well. The problem is that no matter where i set the rod adjustment the clutch wont fully disengage and is slipping. If i adjust it to fully disengage immediately off the floor it will slowly over time get to the point where it dissengages clear up at the top and then it will start to slip again. I put brand new act 2600 pressure plate in and street disc in along with new pivot ball about 3,000 miles ago. I can't figure it out. Any input would be highly appreciated.
