Still can't figure out this terrible hesitation

So, every time I go WOT in third gear I get a nasty hesitation.
I've logged the car and IDC is around 50%, air/fuel is perfect to the tune at 10.5
Last time it made a big cloud of smoke out the exhaust and ruptured a hole in the exhaust I had fixed. The hole is right around where the rear o2 sensor is.

Does this sound like somehow the exhaust gases are not getting out fast enough? Would this cause a problem? I have a 2.5in DP and a 3in exhaust. EVO3 exhaust mani and 7cm2 housing on my 14b along with a ported 2g o2 housing.

Also, the car is only reading 2 knock at 100% throttle.

I can post a log if needed.
