2G Bad Pressure Plate? Bent fork?

Lately the clutch has been engage/disengaging lower and lower towards the floor. It doesn't seem to be slipping but it also is jerky getting on/off throttle and at low speed. I checked the fluid reservoir (full), the clutch pedal adjustment nut hasn't moved, and the slave rod is moving. The pedal tension feels the same but I can hear the bearing noise go away almost to the floor where the clutch disengages. The only clutch i've done is on a tractor but to me it seems that it's either a bad pressure plate or bent clutch fork. Is there any way of checking either?
 
Have someone push it in and take a comparison picture if the fork isn't pushed over I would personally replace both slave and master cylinders and bleed the whole thing. Mine was doing the same thing and ended up replacing master, still had issues so replaced slave too and went away. I had no visual leaks but fairly sure slave was bad. Have someone help with adjustment of clutch pedal after if you are unsure.
