Spray some PB blaster and let it loosen up.

I got it off yesterday, it was full of rust in the pivot, but now I found out it is missing one of the nylon bushings/washers and the bolt to take off the pivot is stuck on (it turns but will not come up/out) how do i get it off to put in an extra bushing?
 
Not entirely sure of what bolt you're talking about. It's more of a stud that rotates in the pivot. Correct word is pivot shoulder. If you're saying the actual shoulder is seized to the pivot but is still able to spin, I don't see why you can't rotate the pivot shoulder and remove it from the shifter. After that just put the nylon washers in between the shifter and the nut on the other side of the pivot and reassemble. Here's a diagram.
