how do they get the muffler to come out and point up and to the left

They (who?) probably have an aftermarket exhaust, cat-back/turboback/axleback. An aftermarket exhaust, depending on the brand, as a slight angle.

Well ussually those exhausts are real straight and then make a quick upward turn... My first 3in exhasut was one solid piece that connects to another piece that is on an angle then my can went on the end... It was a pretty good angle...

so are you just asking how they do that? it is just how the piping is bent, but the apexi n1 or even the hks are both like that.

a lot of the ebay exhaust are similar as well.
