In addition to these things, there's also the closed throttle position sensor (or idle switch) that tends to go bad. You can check this on a logger or with a multimeter. With a multimeter, the tab on top of the switch should show low resistance to chassis ground when the throttle plate is fully closed. Also, if the throttle body has been rebuilt or messed with, this sensor could be set incorrectly and be holding your throttle plate slightly open.
