Mis-aligned clutch fork?!

It seems as though my car can never have any normal problems. It always gets the weird ones that no one has heard about. Anyways, I was driving home from school when I go to push in the clutch and all the sudden the pedal gets extremely hard and it's as though I'm going to push the throwout bearing through the pressure plate fingers if I push any harder. I manage to get the car home rev matching and only grinding once. Now it appears as though everything is fine except for the fact that my shift fork is no longer in the center of that square where it protrudes from the transmission. It is completely center horizontally like it should be, it's just offset vertically pointing more upwards toward the hood. This is causing the slave rod to move at an extreme angle which I think is the cause of the hard clutch pedal. I've checked tranny bolts, slave cylinder bolts, transmission to see if it was lined up properly. Everything is fine except for the fact that it seems the input shaft has shifted and caused the shift fork to move with it. Has anyone heard of anything like this at all?

clutch fork isnt bent. Also i replaced that and the pivot ball recently. So its not either of those.
 
Not seen that one however you will probably be pulling the trans. Something has come loose and probably is causing the fork to exert pressure on one side of the throw out bearing which in turn will cause it to hang up on the nose cone which makes clutching very hard.
 
yea i kinda figured i was going to have to pull the tranny once again. But, whats bothering me is how in the hell did this happen. Also i forgot to mention that i now have some clutch pedal freeplay.
 
well i finally got around to pulling the tranny and found out the throw out bearing was seized up and melted the plastic sleeve.
