First REAL dsm problem

So, magically my car is messed up. Yesterday everything was perfectly fine with no signs of any problem. Today I drive to go run some errands and I am wanting to switch lanes so I go about 50% throttle and my RPM's just climb like it is in neutral but I am not moving anywhere. I was near my destination so I kind of just shrugged it off and didn't figure it was much of a problem. When I got done and got back in the car I wanted to drive it around a little more to see if it happened again. 1st gear is completely fine, but once I hit 2nd the RPM's climb without moving. I go to 3rd gear and see if it happens there and sure enough it does. I could barely pick up speed to get on the freeway because everytime it got to 4k RPM's it started revving. My AFC is also indicating that throttle% is 0 when in actuality I am almost at WOT. Has anything like this ever happened to anybody else? I basically have a fast car that is unable to go fast and it sucks really badly.
 
I bought the car in March so I can't say with 100% certainty what the history of the clutch is. From what I was TOLD by the previous owner the transmission, flywheel, and clutch were all installed 3,000 miles ago.
 
I would get a new clutch. Also replace the rear main seal. That might have leaked and the fluid contaminated the clutch. Either way it is a good idea to change it so you won't have to take it back apart if it starts leaking.

i would go ahead and replace the clutch. get yourself something better than OEM like a stage one or two. i got mine off ebay and its been working perfect for over a year now i also replaced the master cylnder and slave to be sure.
