This isn't that big of a deal for me because I had a 6 bolt ready to go in, but it happened 1 week after I bought the car.

The car had a bad harmonic balancer which in turn wore a hole in the timing belt cover. Because the pulley was bad, it wobbled and broke the A/C belt which went into the timing belt cover causing a little timing issue. This happened at only about 15mph.

Mine actually split the piston. It made it nearly impossible to get the piston out, as it was wedged up in there, had to beat it out with a hammer and a solid piece of rod.
