420A Help Diagnosing Clutch Issue/Possible Fix?

Hey guys, I am not sure what I am dealing with here. First off my car is a 1995 Eclipse RS w/ 5-spd, virtually everything stock. For the past few days I noticed it getting harder to shift gears while driving. This morning I went to shift into first with clutch pedal fully pressed in and the it wouldnt shift i to first but car would lurch forward during attempt. Seems like clutch does not fully disengage. I checked fluid resevoir and it was almost empty. So i added fluid and it didnt help at all. I then checked about adjusting clutch pedal and saw that there is a leak where the piston pushes through the seal. Any ideas what the issue could be exactly and how to fix it? Any help or advice would be greatly appreciated. Thanks in advance!
 
^^ second that you will need a new master and possibly a reservoir dep on what shape it is in. Then comes the task of bleeding it correctly as well.
