Can't go past 12lbs and car cuts out badly

I was driving in my friends car today and when under WOT we cannot get the car top boost past 12 lbs. We just finished installing a fmic on the car with a type s blow off valve and we can't figure out where to hook the second vacuum fitting to. Could this be the problem? Any help would be appreciated
 
The second fitting (The bottom one I assume) is for the quick release feature but it is more likely to leak boost with it hooked up. I would leave it open to the atmosphere. What model is it?1g or 2g? Stock turbo or aftermarket? Could the turbo possibly be on its last leg?
 
1g awd, fp sleeper 16g where should the top brass looking fitting be connected to?
 
If your talking about the Greddy type S BOV the only place I have hooked up is the side one off of the intake manifold, like in the picture. I don't believe the bottom one is suppose to be connected with anything. I think you have a boost leak somewhere if you can't build more than 12lbs, especially with all the new piping and such... do a boost leak test to make sure.
