I got this low mile ACT 2600 plate and dont really know what its worth or if its even worth keeping around. Everything worked great until the TOB blew up. Keep in mind pics are of it fresh out of the garage all gunked up.

But a new one is only like $40 at cost. So if you have a hook up on auto parts, then just get a new one.

An ACT 2600 PP is $40? Where? I'll take 30 of them.

I'd check for warpage with a straightedge. If it's reasonably straight, I always just clean up the surface with some 80 grit sandpaper on a palm sander, works fine.
 
I just replaced my clutch last weekend... it was completely shot. The clutch disk itself was so worn down the tabs were sticking out which lead to extreme grinding against the pres plate. I dont know how I made it 300 miles home... or how it lasted even that long. Now I have a brand new clutch and master so I wont have to worry about that for sometime. Next is the timing belt
