Clutch Gone?????

I was wondering if the symptoms I'm experiencing with my car is a toasted clutch.. First off it's hard to put the car into 1st, even with the clutch to the floor which is the only time I can put it in, then it clicks twice into it.. Also when its in first and the pedal to the floor the car will move foward when I rev it to around 4500 rpm. Also it is needs almost full pedal throw to put it into other gears sometimes grinding. What's up with my car, it really sucks to dirve and seems to be getting worse over time.

actually the clutch sounds good, you need to adjust it to get it off the flor and diengage sooner, try the www.vfaq.com for the directions.
