Loose/Bent clutch fork?

Ive been having a few clutch related issues lately and im hoping a new clutch fork will fix them.

1. with the clutch engaged, i hear a steady rattling noise in the lower driver's side of the engine.

2. every so often (mostly when it's cold out or i havent driven the car in a while) ill start moving from just having gone into first gear, the rpms will be maybe 1300-1500 and it will feel like the clutch isnt gripping like it should. ill be at maybe half throttle during this and suddenly the car will lurch forward and ill get full power. this will only happen maybe once or twice when i start the car, then it will drive normally.

some background: 7 months ago i had a performance shop put an ACT 2600 in (with resurfaced flywheel). 4 months ago i replaced my master/slave cylinders. i havent really been "beating" on the car lately, the problem just kind of came out of no where. the car has 100k miles on it.

ALSO: i don't grind going into any gears, ive read this is an issue with bad clutch forks - thought id throw that in.

Did you put in a new throwout bearing? My problem was somewhat similar and it was because of a bad throwout bearing
 
still on the stock TOB. ive read a million threads about people having bad TOB's because their clutch would rattle when NOT engaged and would stop rattling when the clutch was engaged. my problem is just the opposite and im wondering what it is exactly :(
 
i just did a few days ago actually.

oh yeah, the rattling sound increases as rpms increase, if that helps at all.
