Yeah, sounds like the throw out bearing but you may need to give a little more info on when it is hard to shift. The clicking noise when you let the clutch out is definatly the TOB, but the shifting thing could be a couple of things.

As far as the bolt to put tranny fluid in, I would suggest just picking up a Haynes manual from AutoZone. I really don't like them, but they should be able to at least get that right.

A couple of things. Possible tob but can also be the countershaft bearings. Does the noise exist in all gears but fourth? If so then this would explain the hard shift. I've replaced several trannies due to this. It's a weak point in the F5m22 trans. This means front wheel drive manual 22 series trans. You can replace the bearings and if fourth is quiet I would do it soon before the gears are damaged beyond repair. The fill plug is in front of the trans about midway up and is 17 millimeter. Sometimes they are real darn tight. Use a 6 point good quality socket. Also, if the trans is ok and it's not bearings you can use GM synchromesh to help with the shifting. Mitsubishi used to sell a shift enhancer years ago when I worked for them. I am not sure if it is still available as I think shifter block was cured when they went to the 2g type trans.
