Dsmlink log Is my O2 sensor dead

I think my o2 sensor died. I made sure that it was plugged in and all the wires are fine but nothing. The front o2 is not reading anything or maybe I'm not logging the front o2 can someone verify this. My car is also leaning out like crazy even after adding alot of fuel so I'm pretty sure the o2 died.

Forgot to add that dsmlink dtc says "heater circuit malfunction". I did a search but didn't find an answer. I read up on someone that changed his 02 several times, He tried a used 02 and new o2 sensor and had no luck. Does anyone know what the heater circuit malfunction is, maybe I'm missing a fuse somewhere.
