Window Help (off track)

My driver's window always seems to go off track and I cannot or rather don't have the knowledge on how the tracking system on a window works.

PROBLEM:

when almost 75% up the window seems to lundge forward causing there to be a gab at the back up most part of the window.

If anyone knows how I can fix this easily or what maybe causing this I am all ears
