killed a 14b, but why?

Tonight I verified that my 14b is now dead, I looked at the compressor wheel and it is shot and turbo has a lot of shaft play (sounded like a supercharged lightning when it spooled). I want to know why though. The turbo doesn't have coolant lines going to it, it has a NT coolant pipe which i guess is where the feed or return goes. I bought the car like this (cheap). I have heard conflicting reports about this hurting turbos. I took the turbo off when it had been run like this for a while and it had almost NO shaft play at all. Could it be possible I have an oiling problem? I always let it run for 5 minutes before shutting it off after driving. How can I check if my turbo is getting enough oil? Any help would be appreciated. Also, what should I do before putting a new turbo on? Clean the intercooler of any compressor wheel debris?
 
ditch the NT water pipe, so you dont cook yor new turbo aswell.

or get a ball bearing turbo.
