Myths and Preventions of Crankwalk

I've heard alot of myths about crankwalk. I think it's talked about more than it actually happens. Some things I've heard

-95-96 DSMs have a higher chance of crankwalk
-ACT 2500 and 2600 clutchs raise the probability

Can someone once and forall clear up these myths/truths and list some preventions of this feared problem

I favor the magnus block/cooling squirter feed idea since there was a revised block in 98.

oh and prevention, change oil regularly and never start the car with your foot on the clutch.
