The seals in your turbo could have went out. That happened to my stock turbo and i replaced it and it quit smoking. But im sure it could be something else too

Andy's idea is definately the easiest, and probably the best, but to make it a little bit easier, you should take a torch to the TB and heat it till it starts to glow a little, and that will expand the metal, and make it a lot easier to take out.

I have the same shifter. That "unknown" piece is a bearing. It does fit into it, but you'll have to beat it in with a hammer. Once you get it in far enough to start the nut onto the bolt, you can just screw it the rest of the way in.
